Only other one that springs to mind is the Beyonder, he always seems to be missed off these lists. Cannot remember the exact issue but sure someone here will.

Then there are also those heroes who were rejected for membership, but thats a different kind of list.

Spider-Man. I believe he was asked a couple of times before accepting the reserve status, most notably early on when they sent him to find the Hulk as a test.

The Hulk mission happened in Spider-Man annual number 3. It could be argued that he tacitly refused membership by intentionally not completing the mission out of sympathy for the Hulk. From the Avengers point of view, he would look like a rejected candidate.

Also, Namor refused membership in Avengers 16, but of course later joined.

In one of the Avengers Annuals (1999? - it was the one where Fabian went nutso and had Sentinel Avengers roaming the streets), it was proposed that Jarvis be made an official member, but he vetoed it flat out before it could go any further.

Those guys call and call begging me to join, and I have to tell them no again and again, it's really annoying. I tell them I'm really a loner, but do they listen? Nooooooo. Captain America was the worst. "Poltargyst, help us save the world again." Sheesh!

The Thing - WCA #3-#10, I think he holds the record for most refusals since Hawkeye asked him about 3000 times over a seven issue span.

Daredevil turned down the offer to Cap numerous times. Just check NA#1
