Mark Millar and Brian Bendis had this interview with each other (more like Bendis interviewing Millar by phone) for Wizard magazine and Millar said he did have a sequel in his head but Marvel wanted Civil War to be much bigger than his original pitch.

His most reucurring image regarding Civil War is himself on a hospital bed, puking several times and trying to finish the script. SO he probably will not want to visit it sometime soon.

When the "Order" inevitably fails, the old order changeth. IronMan's Initiative agenda will collapse(someday). This means, the post-CW status quo will become the Post-post-CW era. Of course, a Major-Super-Mega-Epic-Crossover Mini-Series Event will be need to make the story(money) possible.

Is that a sequel? maybe more a conclusion.
Here's hoping for "The Return" vol 2. in a few years.
