Did he ever die, if so, was it at anothers hand?

It could have been by anybody's hand that he dies yes, though if you wanted to stretch the point this could be a muddled memory of the events of Grant Morrison's Action Comics run, where time and space are being rewritten by Vyndktvx and history altered in various ways.
For instance it was Vyndktvx who was responsible for the deaths of the Kents, a fact that still stands - Vyndktvx's meddling altered what should have been.
Superman dieing shouldn't be a surprise, after all he died all the time back in the Silver-age, and even in the early days of the John Byrne revamp he 'died' at the hands of the Silver Banshee and was presumed dead several times over...

My idea would not meet DC approval, but always thought that it would be neat if New earth Supes had died battling that DD, so acxtually history would have been the same up to that point, and then His resurrection would have been as young kal, ushering in the NU 52, that the Spectre or Presense had redone reality!

I don't know what to say about the subject when the publisher is firmly sending out two completely opposing views on the question.
The original thought behind keeping Death of Superman in-continuity was apparently based on it being such a monumental story; though since this is a whole new canon and reboot that urge to keep it as a part of the new history makes no sense in practical terms. It would have made more sense to just retell the story as a part of the new ongoing narrative, which to a degree is what Superman:Doomed is all about.
But since Doomsday is such a one-dimensional character, who's entire impact and importance stems from that now deleted 20 year old story where he was Superman's ultimate challenge... well, he's like the new Superboy - What is the point of him? \(euh\)

His ONLY use would be as the one able to best Superman, as the one enemmy who would be more then a physical match up for him!
