Cap wasn't exempt, he simply lived on after World War II ,became a guest speaker and war hero for the next sixty years. Then he became an astronaut,a politician and ultimately a vocal foe of the state. He is walking down a street (as an old but still fit man) and the last we see of him is that a group of human thugs are about to mug him.

Actually, those were the heroes who remembered the House of M reality (Spidey, Cage, Emma Frost, etc.) and had tried to recruit him earlier. I'm guessing they went back to get one last look at him (for inspiration, perhaps) before going off to fight Magneto. \:\)
