He is an erratic fighter at best, but he has occasionally been shown to have the strength of a madman (surprise, surprise) and his random way of fighting actually makes him harder to predict the moves for Batman.

Not in any sane way. He see's Batman as the only "real" person in the universe other than himself (Robin, Gordon etc are all toys to play with, but aren't "real"). Batman is the only other player on the chessboard

Essentially yes, he's human. "The Last Laugh" crossover had doctors discuss the fact that the Joker's brain was physically deformed past the point of any hope of rehabilitation, and one story had his blood as toxic, but most other stories seem to emphasise the fact that the Joker is as human as anyone, and that makes him scarier IMHO, since making him a pure monster isn't nearly in an easy out, and not nearly as scary as the notion that he's a "normal" guy capable of committing the acts he does.
