He was always prepared to kill

In his first issue, his appearance under the mask was never a plot point. We saw him before he first donned the mask.

Not that I recall, though in one of the Animated comics, he escaped from Arkham and, under an assumed name, restarted his teaching career and would seemingly have been content to never become the Scarecrow again, until the rape (not mentioned as a word, but the inference is clear) of one of his promising students (a shy but brilliant girl) led him to seek revenge on her attacker (a handsome popular jock) as the Scarecrow. A great and disturbing little story that one, as the police lead the Scarecrow AND the rapist away, Batman comments that though the Scarecrow may be more dangerous than the rapist, at least he's always had the decency to wear his mask on the outside when committing crimes.
