He cares in that he NEVER wants to find out. He's threatened and attempted to kill those that do know in the past to keep the secret for Bruce, since it being revealed to anyone, ESPECIALLY him, ruins the fun of his match with Batman.

That said, I choose to read his conversation with Batman in the Hotel Room during Death in the Family as him having figured out that Bruce was the Batman, ("Even a madman can put two and two together", to Batman upon talking about his killing Jason Todd, followed by his insane laughter upon seeing Bruce Wayne at the UN.) but, since this was an unusually lucid period for the Joker, that he's long since forgotten that he even once did know.

(Joker actually worried about the state of his finances and tried to cover up that he beat Jason nearly to death... that's pretty sane. Around the same time, he also enters into an assassination plot against Batman's civilian ID with the Biyalan Secret Police that was WAY too sane for the Joker. Enough that, upon presenting this evidence, I'm not even sure that WAS the Joker anymore...)
