I bought it last night and enjoyed it.
I wouldn't go as far as IGN.com does and call it one of the classic, must-read Joker stories ever written (like say The Killing Joke or the Five Way Revenge story), but it's certainly a solid, fun read.
Not to go into specifics, but it doesn't take place in the Batman Begins/Dark Knight movie-verse nor is it in the mainstream DC Universe. The Joker's physical appearance is very similar to the Heath Ledger Joker, but his personality is somewhat different. It's told from the viewpoint of a random no-name thug who becomes the Joker's right-hand man.
The art is gorgeous and I thought the writing was good stuff. I think people should at least check it out before they buy. Right now the hardcover is $20 for roughly 120 pages of original story, no extras.
A warning that the Joker does some very brutal, graphic, sadistic violence in here. It's not particularly gory, but it may not be for the weak of stomach.

I would say most of these reviews are highly biased with no objectivity.

One thing all these reviews should start out with is calling Joker an Elseworlds publication taking place in an Azzarello Earth. Following this, the reviews should state that the characters in the Joker including the Joker are brand new Azzarello creations that masquerade as but are not the classic characters of the definitive Batman comics.
