I mean, has in any of the previous(before cap issue # 25)storylines in cap's own book shown any at all foreshaowing of his death? Has it been so quietly Ever hinted at? I understand this idea was made a year and a half ago to kill him off. Good writters, in story telling, usually Do use foreshadowing of such a very serious event. In the frontline comic#11, someone declares that steve has no clue what real america is really about and that his dream is already dead(he just never knew it).
He Does Now! Now he may become not the american dream, but the american Reality! No longer a symbol of american pie, now, a symbol of american Survival in a more dangerous world than ever existed before! We can be hurt! We can be bleed! We can die! His eyes are wide open now! You saw the change of bucky into what he is now(winter soldier). Now it is cap's turn! He was out of touch and we were out of touch with him. After this; we will both be on the same page, fighting the same fight; for the same things: survival!
Some will say America was hit in the heart with 9/11(a very real eye opening tragedy that has changed america forever), so too is cap The heart of the marvel universe. Changes Must come. 9/11 did it for us- This will do it for Cap*

Yeah, the Red Skull's revenge been an undercurrent throughout the series, which has gradually been building steam. Since the relaunch part of the stories have revolved around Steve trying to find Bucky, as Bucky hunts down Lukin/Skull. Simultaneously, Lukin/Skull have been putting pieces in place to strike back at Cap ... and this become much clearer during the Civil War tie-ins.

So yeah ... it has been hinted at. The timing was "right" for the issue, given the Civil War finale ... but if you've been reading the book since issue 1 you'd probably realize that Bru's been building towards this. That's why most of the regular readers are upset by what happened, but not screaming about how "evil" Marvel is or how they'll never read another Marvel book. It's obviously another part of a broader story.
