The fact that he's repowered and one of the most powerful beings on the planet, as well as cursed to become a threat to that world, which is a situation that he doesn't like by the way, he's someone that the Initiative can't ignore and that would make an awesome story where he fights their forces in a area, maybe an island with hardly any people in it for Cain to think, where he shows them that "Nothing can stop the Juggernaut!" which puts even Iron Man in disbelief in his attempts to stop him, even with those nanobots to turn off his powers which would probably fail because of his nature.

He hasn't been much of anything since Claremont stopped using him as a villain in the 80s. Marvel's attempts to rehabilitate their villains--and we've seen much too much of that--usually means they lose their traditional personality altogether and simply become Bland White Bread. The latest example is the Constrictor in The Initiative.

Marvel needs MORE real, viable criminals with teeth, not less--drastically so.

Look how easily--of course--the Frightful Four/Five lost to the Fantastic Six in the current FF arc. The Trapster hardly even appears, and doesn't in the current issue--he literally 'may as well not be there,' because he isn't.

Look how the Wizard, Madame Masque, and the Crimson Cowl were shown to be terrified of the fact that the Owl ostensibly took out a contract on them---in the MU of the past, Madame Masque and the Wizard wouldn't be intimidated by such an action, especially the former.

So unless Juggie is Once & For All Time established again as one of the most powerful threats in the MU, and is infrequently used as such, he might as well dribble along.

Marvel keeps creating these 'ultimate threats' like the Exemplars or the Collective that then turn out to be essentially panty-waists, which is why Annihilation was so refreshing. On the Good-Guy side, the Sentry suffers from that too.

Austen's getting a hold of Juggie, of course, didn't help.
