According to OHOTMUDE (1985) Myron MacLain began on his work with steel alloys that would eventually result in the creation of Adamantium in the 1940s, and in one experiment he tried to fuse his experimental steel alloy with a sample of Vibranium, which accidentally succeeded when he wasn't watching, as you described, and which was made into Captain America's circular shield. In the years following this MacLain tried to replicate the process and eventually "in recent years" created the metal called (True) Adamantium, which is nearly as strong as the metal of Cap's shield ("unknown iron-Vibranium alloy") but does not contain any Vibranium (True Adamantium first appeared in Avengers #66 (1969), as the metal of Ultron's outer shell).
The secret of making True Adamantium was shared by the US government with some of its allies and also stolen and come into the hands of some criminals, e.g. the Constrictor. However, because True Adamentium is so difficult and expensive to produce, "certain parties" have created a cheaper and somewhat weaker alternative which the OHOTMUDE calls Secondary Adamantium. This can e.g. be destroyed by Blastaar's energy discharges and the Overkill Horn.

According to OHOTMUDE, the following are made of True Adamantium:
The Constrictor's weapons.
Ultron's casing.
Wolverine's claws and the metal parts fused with his bones.

The following are or were made of Secondary Adamantium:
The casing of F.A.U.S.T. (destroyed by Thor in concert with Iron Man)
The dome protecting the island fortress of the Red Skull and the Hate-Monger (destroyed by the Overkill Horn and incendiary bombs).
